Provision of Services

Pick the classes your group wants to join, fill out the form, and send it to the Camp U-Nah-Li-Ya OEE Director. Make sure your dates are confirmed with the Outdoor Education Director before filling this out!

Medical Form
If groups do not have their own Medical Release form, they can use the form attached and it must be filled out by the parent/legal guardian of all minors attending camp.
Rental Group Policies and Procedures
These policies and procedures must be reviewed and followed by all rental groups visiting YMCA Camp U-Nah-Li-Ya.
Challenge By Choice
For groups that are participating in our rock climbing or high ropes course, each student must fill out this form with their parents prior to their trip to camp.
Student Participation Form
Parents/legal guardians of all students attending camp must complete the student participation form prior to their visit.
Financial Assistance Scholarship
Families needing financial assistance to send their student to camp can apply by completing this form. Scholarships are only available if the families are paying the camp fee—we’re here to help make camp possible!
Packing Lists
Whether your student is visiting Camp U-Nah-Li-Ya for an Outdoor Environmental Education day trip or overnight trip, these packing lists will provide suggestions of things students should and should not pack for their visit.
